Bridging AI and Primary Care: AI-assisted Health System Integration Model for the Inclusion App

Rehabilitation in Rwanda remains limited to higher-level facilities, leaving rural populations underserved. To address this, the Inclusion App has enabled community health workers (CHWs) to deliver basic rehabilitation since 2023, benefiting over 13,000 people and reducing costs significantly.

To overcome remaining challenges such as limited expertise and irregular follow-up, the team introduced the AIRe platform, which integrates AI into the health system. AIRe consists of AIre Talk, a conversational tool for users providing personalized guidance, and AIRe Hub, which lets professionals customize content easily without coding. The system supports continuous learning and culturally relevant communication, including Kinyarwanda.

Pilot testing showed that AIRe is easy to use, accurate, and empowering for CHWs, helping them provide immediate, reliable advice for conditions like arthritis, back pain, and mental health concerns. The model emphasizes interoperability and equity to prevent technology-driven inequalities, offering a blueprint for sustainable rehabilitation integration in low-resource settings.
